Athletic Clearance Form & Instructions for 2025-2026

  • All student athletes must complete a physical and show proof of insurance as defined by CIF.
  • All forms must be completed before the athlete can practice or represent AHS.
  • All participants must complete the following clearance procedures as outlined below.
  • All pre-participation physicals must be completed by one of the following:
  1. Medical Doctor (MD)
  2. Doctor of Orthopedics (DO)
  3. Nurse Practitioner (NP)
  4. Physician Assistant (PA)

Instructions for uploading a physical to a home campus, including scanning and uploading.

As required by CIF Bylaw 503G, all athletes competing on an athletics team, band, cheer, color-guard, are required to receive an annual preparticipation physical examination (PPE) certifying that the athlete / student is physically fit to participate in athletics. The physical must be completed and turned in via Home Campus portal found on the Arroyo HS athletics website prior to any tryout, practice, or competition. Hard copies of the forms can be found in the AHS Activities Office, or the Athletic Training Clinic (Room 40).

All physical examinations must be completed and signed by an MD, DO, PA-C, NP. We are unable to accept any physical examinations signed by a DC (Doctor of Chiropractic). Physicals can be completed with your primary care physician, urgent care or during campus sessions during scheduled times throughout the year.
